Repatriation of Human Remains to Trinidad & Tobago
The following documents are required for the repatriation of human remains to Trinidad & Tobago:
- Three (3) original copies of Death Certificate;
- Three (3) original copies of of the Medical Certificate of Death;
- Burial Permit;
- Certificate verifying the embalming of corpse in accordance with health regulations;
- A letter from the Health Department stating that there was no communicable disease at the time of death;
- Certificate for shipment of a body outside of the Province of Ontario;
- Letter from the Board of Funeral Services indicating that the Funeral Director's Licence is valid and in good standing;
- Trinidad and Tobago passport of the deceased;
- Details of airline arrangements;
- Name, Address, Phone and Facsimile number of Consigneee;
- Letter from the funeral home indicating travel arrangements (Airline schedule and the name, address and phone number of person and or funeral home receiving the remains);
- Money order payable to the 'Consulate General of Trinidad and Tobago' in the amount of CDN$20.00.
- A Letter from the Consulate General/Embassy confirming the authenticity of the abovementioned documents for the transportation of the human remains.
Requirements for travelling with cremated human remains to Trinidad and Tobago:
- the remains must be enclosed in an urn or in another receptacle with a lid that is cemented or sealed;
- the receptacle must be wrapped with packaging material; and
- the bearer of the package must have a death certificate and a certificate of cremation issued by the appropriate authority which must be enclosed in an envelope and attached to the receptacle.
